Disability
Services
In accordance with the Americans with Disabilities Act, it is the policy of Clark State Community College to provide support services to persons with disabilities. Disability Services is the official contact for students with disabilities who request academic adjustments, reasonable modifications, auxiliary aides and/or services to provide equal opportunity for academic success.
Students must self-identify (PDF) to Disability Services, provide documentation (PDF) of their disability from a licensed professional and sign an Authorization for Release of Information (PDF). The Disability Services Office serves as advocates for students with disabilities and assists them in achieving equal access to all college programs and services. Typical accommodations offered are alternative testing accommodations, taped textbooks, sign language interpreters, assistance in securing volunteer note-takers and reader/scribe services.
If you are a person with a disability, it is important for you to contact Disability Services before the academic quarter so that arrangements can be made to assure your access and to meet your special needs. Services are provided based upon your individual need.
If you would like more information about how to request these services, please contact Disability Services on the Leffel Lane Campus, Rhodes Hall, Room 215B or call 937/328-6019.
In compliance with the National Voter Registration Act, Clark State provides voter registration forms and assistance in completing them in the Disability Services Office and the Records and Registration Office.
If you have a documented mental or physical disability, you may be eligible for Student Support Services.
Handicapped Parking
If you require handicapped parking and do not have a State of Ohio permit, please view the Handicapped Parking Permit Guidelines and print and complete the application (PDF). Applications are available in Disability Services, Rhodes Hall, Room 215B. Upon verification of registration and disability, you will be issued a handicapped parking permit. The permit will be sent to you or you can pick it up at the Disability Services Office. The permit expires when indicated by the physician and/or at the end of the current quarter or school year. The dated validation sticker will be on each permit. You must complete a new application each year, including the physician's statement and signature.
